I, the undersigned parent/guardian (or participant if 18 years or older), give permission for participation in the youth camp organized by Emmanuel Church.
I understand that participation in camp activities involves inherent risks, including but not limited to physical injury, illness, accidents, foodborne illness, and other unforeseen circumstances. I voluntarily assume all risks associated with participation in these activities.
I hereby release, waive, and discharge Crowders Camps, Emmanuel Church, its pastors, leaders, volunteers, staff, and affiliates from any and all liability, claims, demands, or causes of action arising out of or related to any injury, illness (including food poisoning), loss, or damage that may occur during or as a result of participation in the youth camp.
I acknowledge that it is my responsibility to disclose any medical conditions, allergies, or dietary restrictions prior to participation. I also consent to emergency medical treatment if deemed necessary, and understand that I am responsible for any associated medical costs.
By signing below, I confirm that I have read and understand this waiver and voluntarily agree to its terms.
